We report the case of a 26-year-old woman presenting with hyperventilation syndrome as the first neurological symptom. On visiting the emergency room, the presence of apparent anxiety with tachypnea and respiratory alkalosis with hypocapnia suggested hyperventilation syndrome. However, we performed brain magnetic resonance imaging due to the lateralizing focal symptoms of her left hemiparesthesia and visual field defect, which demonstrated an acute infarct in the right posterior cerebral arterial territory that included a thalamic lesion. We considered this to be a rare case of thalamic infarction that first manifested with hyperventilation. (Korean Journal of Stroke 2007;9:86-87)
